PASSPORT TO HEALTHY LIVING

CHALLENGE

A national consultation conducted by the Active Living Coalition for Older Adults (ALCOA) in 2007 identified that many programs for primary prevention of Type 2 Diabetes were not evaluated. No measurement tools appeared to exist for non-clinical community program leaders to measure the success of their programs. It was recommended that such a tool be created.

SOLUTION

In 2009-2011 ALCOA developed a Passport to Healthy Living for use in community programs that focus on the prevention of Type 2 diabetes amongst older adults (age 55+), and management of diabetes among older adults who have Type 2 diabetes. The Quaich provided consulting expertise during the research, development and testing of the Passport and accompanying leader guidebook.
