PASSPORT TO HEALTHY LIVING

ALCOA, the Active Living Coalition for Older Adults, developed a Passport to Healthy Living for use in community programs that focus on the prevention of type 2 diabetes amongst older adults (age 55+), and management of diabetes among older adults who have type 2 diabetes. The Passport has been pilot tested with good results and is available in French and English.

It is designed to motivate older adults and to measure the success of each individual participant in achieving their health goals. The Quaich Inc. Inc. served as consultants for the development, focus testing and pilot testing of the Passport.
